What You Will Need to Complete

The following forms are required for all new patients. Each form plays an important role in helping us provide you with the best possible care.

New Patient Intake Form

Basic demographic information, emergency contacts, and reason for seeking therapy. This helps us understand who you are and how to best support you.

Mental Health History Questionnaire

A brief overview of your mental health history, including any previous therapy or counseling, current symptoms, and relevant medical history.

Consent for Treatment

Informed consent explaining the nature of therapy, your rights as a client, confidentiality policies, and the therapeutic relationship.

HIPAA Notice of Privacy Practices

An acknowledgment that you have received and reviewed our Notice of Privacy Practices, which explains how your protected health information is used and safeguarded.

Release of Information (if applicable)

Optional authorization allowing us to communicate with other providers, such as your primary care physician or a previous therapist, to coordinate your care.

Payment & Insurance Information

Insurance details, payment preferences, and financial responsibility acknowledgment. This ensures a smooth billing process from day one.

How to Complete Your Forms

Follow these simple steps to ensure a smooth experience before your first visit.

Complete Forms Online

All forms are available through our secure, HIPAA-compliant patient portal. You will receive a link to the portal via email after scheduling your first appointment.

Submit Before Your Visit

Please complete all required forms at least 24 hours before your scheduled appointment. This allows your therapist to review your information in advance and make the most of your session time.

Take Your Time

There is no need to rush. The forms are designed to be straightforward, and most clients complete them in about 15 to 20 minutes. If you have questions about any form, our team is just a phone call away.

Your Privacy Is Our Priority

All information you provide is protected under HIPAA (the Health Insurance Portability and Accountability Act). Your data is encrypted, stored securely, and only accessible to your care team. We will never share your information without your explicit written consent, except where required by law.
